- A Valid Card on file is required for All Bookings this is due to our 48 hour Cancelation Policy that will require your signature at the time of your booking.
- You may be asked to set up a VAGARO Account when booking. This is highly recommend for ease of appointment reminders signing of consent forms.
- When setting up your account, we recommend making sure that your email, text and push notifications are on to ensure you receive your appointment reminders & confirmation emails.
- If you do to receive a confirmation e-mail please 1st check your spam then give us a call.
- On your service, we may require some forms, be signed, including consent to treat. We asked that you please have these forms filled out prior to your appointment if you were not able to complete the forms prior to your appointment, you will have time to complete the forms when you arrive.

GFE Requirements For Med Spa Bookings
It is the California medical board and state law that All clients who wish to receive any Medspa services, including but not limited to Laser, hair removal, Micro needling, Botox, Fillers, I.V. Infusions and Vitamin Therapy are require to have a GFE a.k.a. a Good Faith Exam performed by a Nurse Practitioner Prior to receiving treatment.
- Our Nurse Practitioner at Simply Me Skin Care & Med Spa is Dr. Mike, D.N.P.
- Dr. Mike, D.N.P. will send you a text message so you may coordinate a day and time for your Telehealth Video Exam (Approximately 5min). This exam is for your safety and is necessary to “Medically Clear you for your service." Most clients will only need to receive a GFE once per year. Your GFE must be completed prior to your appointment and may be done remotely or once you arrive on the day of your appointment.
- During your Exam You will be discussing Medical History, Allergies and Past Treatment History. You may also Receive Important Pre & Post Care Instructions.
